Overview
Sometimes, the big toe can become angled outwards towards the middle of the foot and second toe. This forces the top of the first metatarsal to stick out from the side of the foot at the base of the big toe. If this happens, the bones can become misaligned and a painful bunion can form. It is not known exactly what causes bunions, but wearing badly fitting shoes is thought to make the condition worse. Research also suggests that bunions may run in families. It is thought that bunions are more likely to occur in people who have unusually flexible joints, and that this flexibility may be inherited. In some cases, certain health conditions, such as rheumatoid arthritis and gout, may also be responsible for the formation of bunions. These conditions cause pain and inflammation in the joints.
Causes
Bunions tend to run in families, although it is the faulty foot mechanics that lead to bunions that are inherited, not the bunions themselves. Some authorities, in fact, suggest that the most significant factor in bunion formation is the poor foot mechanics passed down through families. However, the American Orthopaedic Foot and Ankle Society estimates that women have bunions nine times more often than men, that 88 percent of women in the United States wear shoes that are too small, and that 55 percent of women have bunions. Again, this reflects the wearing of shoes with tight, pointed toes, or with high heels that shift all of your body's weight onto your toes and also jam your toes into your shoes' toe boxes. It should be noted that it generally takes years of continued stress on the toes for bunions to develop.
Symptoms
If you have a bunion, you may have pain or stiffness of your big toe joint, swelling of your big toe joint, difficulty walking, difficulty finding shoes that fit. These symptoms may be caused by conditions other than bunions, but if you have any of these symptoms, see your doctor.
Diagnosis
Generally, observation is adequate to diagnose a bunion, as the bump is obvious on the side of the foot or base of the big toe. However, your physician may order X-rays that will show the extent of the deformity of the foot.
Non Surgical Treatment
Detecting and treating bunions can relieve many of the symptoms associated with this condition. Doctors often consider whether the condition requires non-surgical or surgical treatment. The decision is based on the severity of the symptoms. Because bunions often get worse over time, early detection and proper treatment are very important. Some non-surgical methods to reduce the symptoms related to bunions include cushioning the area with padding or tape, taking medication to relieve pain and inflammation, using physiotherapy to reduce pain and related symptoms (e.g., ultrasound, whirlpool baths, joint mobilization), wearing custom orthotics to provide better movement and stability of the foot, wearing well-fitted comfortable shoes that are not too tight (if your shoes used to fit but now are too tight around the bunion area, you may be able to have them stretched in this area), using a special splint at night to decrease the amount the toe angles towards the other toes. When non-surgical methods do not provide relief, surgery may be needed. During surgery, the doctor will remove the tissue or bone in the area of the bunion and attempt to straighten the big toe, and may join the bones of the affected joint.

The surgical operation to correct the deformity from a bunion is called a bunionectomy, which typically involves removing bony growth of the bunion, re-positioning ligaments and tendons, and realigning the bones of the toe joint. Surgery is usually a day procedure performed with a local anaesthetic. The bones may be stabilised in their new position with screws or pins. Hardware may even include absorbable pins that are broken down by the body after a few months. You can expect a 6 - 8 week recovery period during which crutches are usually required. Surgery is often successful but sometimes the big toe moves back to its previous deviated position. Proper footwear and orthotics reduces the chances of surgical failure.
